Until it has been, the notice stays on every page and cannot be turned off, including by subscribers. Your own observations and conclusions are recorded in full and stand on their own; the derived rating categories should be read as provisional.
No. You enter the likelihood of failure, the likelihood of impact, and the severity of the consequences. The app combines them and prints how the rating was reached, so the reader can follow your reasoning. It does not inspect or diagnose anything.
Three in total, not three per month. Reports you have already issued stay on your device and can still be opened and shared whether or not you subscribe.
Yes. Recording, photographs, GPS coordinates, and PDF generation all work offline. The one thing that needs a connection is turning coordinates into a street address; the app records the coordinates in the field and fills the address in later, or you can type it yourself.
Either location permission is off, or the device could not get a fix in time — under dense canopy this happens. The photograph is still attached; only the coordinates are missing. You can check permission in iOS Settings → Privacy & Security → Location Services.
No, and this is deliberate. An issued report is frozen, and a checksum of the PDF is stored with it, because these documents can be referred to long after the visit. To correct something, create a revision — the original stays as it was, and the revision records what it supersedes.
Whatever is in your most recent backup file can be restored to a new device. Export a backup from Settings → Backup and restore, and keep the file and its passphrase somewhere other than the phone. The passphrase is not stored on the device and cannot be recovered by anyone, so a backup without its passphrase is not recoverable.
In your Apple Account settings, under Subscriptions. Deleting the app does not cancel a subscription.
